This study is designed to evaluate the safety and effectiveness of a new treatment for individuals with Extensive Stage Small Cell Lung Cancer, which is a type of lung cancer that has spread to other parts of the body. The research aims to compare a combination of the experimental drug ABBV-706 and atezolizumab against the current standard treatments. The standard care options being compared include carboplatin and etoposide, which are medications typically given through an intravenous line, meaning they are delivered directly into a vein.

Participants in the study will receive their assigned medications through an intravenous infusion. The goal of the research is to determine the best dose of ABBV-706 to use in future larger studies and to see how well this combination works to control the disease compared to traditional therapy. The study will monitor how long the cancer remains stable without getting worse and how the body responds to the different treatment combinations.

The research process

The trial runs in 4 steps – from screening to follow-up. Each step says what happens and what the team monitors.

  1. Step 1

    Treatment assignment

    You will be assigned to one of two groups to receive different types of treatment for extensive stage small cell lung cancer, which is a type of lung cancer that has spread beyond one lung.

  2. Step 2

    Experimental group treatment

    If assigned to the experimental group, you will receive a combination of ABBV-706 (an antibody therapy, which is a type of medicine that targets specific parts of cells) and atezolizumab (an immunotherapy, which is a medication that helps the immune system fight cancer).

  3. Step 3

    Standard of care group treatment

    If assigned to the standard of care group, you will receive carboplatin and etoposide (two types of chemotherapy, which are medicines used to kill cancer cells) along with atezolizumab (an immunotherapy).

  4. Step 4

    Monitoring and evaluation

    Throughout the trial, the effectiveness of the treatment will be measured by tracking progression-free survival, which is the length of time during and after treatment that the cancer does not grow or spread.

Who can join the trial?

7 criteria

  • You must have a confirmed diagnosis of extensive stage small cell lung cancer, which is a type of lung cancer that has spread widely within the chest, and you must require first line therapy, meaning this is the first treatment you are receiving for this condition.
  • You must have an ECOG Performance Status of 0 or 1, which is a scale used by doctors to measure how well you can perform daily activities; a score of 0 or 1 means you are physically able to carry out most daily tasks and are generally mobile.
  • You must be able to safely receive a treatment plan that includes platinum-based chemotherapy, specifically a combination of two drugs called carboplatin and etoposide, along with a medicine called atezolizumab.
  • Your cancer must include measurable disease, meaning that the tumors are large enough and in locations that allow doctors to clearly see and track changes in their size using standard medical imaging rules known as RECIST v1.1.
  • If you have had radiation therapy in the past, any tumors in those areas can still be measured if they show signs of progression, which means the cancer is growing or spreading.
  • This study is open to both males and females.
  • You must fall within the specific age range defined for this study.

Who cannot join the trial?

6 criteria

  • People with mixed small cell lung cancer, which is a type of lung cancer that contains more than one specific type of small cell cancer cells, cannot join.
  • Anyone who has already received any type of treatment for limited-stage small cell lung cancer, which is cancer that is only found in one lung and has not spread to nearby lymph nodes, is not eligible.
  • People who have previously had systemic therapy, which is medicine that travels through the entire body via the bloodstream, for extensive-stage small cell lung cancer, which is cancer that has spread to other parts of the body, are excluded.
  • Individuals with active or symptomatic central nervous system metastases, meaning cancer that has spread to and is currently causing problems in the brain or spinal cord, cannot participate.
  • People with a history of interstitial lung disease, which is a group of conditions that cause scarring or inflammation in the lungs, or pneumonitis, which is inflammation of the lung tissue, are excluded if they needed treatment with systemic steroids (strong medicines used to reduce inflammation).
  • Anyone showing signs of active interstitial lung disease or pneumonitis on a chest CT scan, which is a detailed X-ray image of the chest used to look at the lungs, is not allowed to join.

  • Carboplatin

The drug works by attaching to and damaging the DNA of cancer cells, which prevents them from multiplying and eventually leads to cell death. It belongs to a class of drugs known as platinum-based antineoplastic agents.

  • Etoposide

    This is a chemotherapy medication that is given directly into a vein through an intravenous infusion. It is commonly used in medical practice to treat several types of cancer, such as small cell lung cancer. At a molecular level, it works by interfering with the enzyme that helps cancer cells package their DNA, thereby stopping them from dividing. This drug is classified as a topoisomerase inhibitor.

  • Atezolizumab

    This medication is administered as an intravenous infusion to target specific immune responses. It is an established treatment in modern medicine used to help the body's own immune system fight certain cancers. The drug works by blocking a specific protein on immune cells that cancer cells often use to hide from the body's defenses. It is classified as an immunotherapy agent, specifically a programmed death-ligand 1 (PD-L1) inhibitor.

  • ABBV-706

    This is an experimental drug provided as a solution for infusion through an intravenous line. It is currently being studied in clinical trials and is not yet a standard part of medical practice. The substance is designed to target a specific protein called SEZ6 on the surface of cancer cells while also carrying a chemical payload to damage those cells directly. It is classified as a specialized antibody-drug conjugate.
