A plain-language summary of the goals, design and what participants do
The study focuses on Knee osteoarthritis, a condition in which the cartilage that cushions the ends of the bones in the knee gradually wears away, causing pain and reduced movement. The investigation evaluates a new medication identified as 4P-004, compared with a simple salt solution used as a placebo, both delivered by an intra‑articular injection, meaning the medicine is placed directly into the knee joint. The main purpose is to assess safety, how well the knee tolerates the injection, and the way the drug is processed in the body, known as pharmacokinetics.
Participants will receive a single injection of either the test drug at one of three increasing dose levels (0.3 mg, 1 mg, or 3 mg) or the placebo. After the injection, they will be monitored for a short period, during which vital signs such as blood pressure and heart rhythm are checked, and blood samples are taken to look for any changes in normal lab values. They will also be observed for any uncomfortable feelings in the knee, such as pain, swelling, redness, or tenderness, and any other unwanted effects that might occur during the follow‑up visits.
The trial runs in 11 steps – from screening to follow-up. Each step says what happens and what the team monitors.

is the experimental medicine being tested in this study. It is a synthetic version of a natural hormone that helps control blood sugar. In this trial, the drug is injected directly into the knee joint of people with osteoarthritis to see if it is safe and well‑tolerated when given at different strengths. Researchers are watching for any side effects and measuring how the drug moves through the body after a single injection.
is a commercially available pre‑filled pen that also contains liraglutide. In the trial, the same liraglutide from Victoza is used as the test medicine and is injected into the knee joint. The purpose is to evaluate its safety, how patients feel after the injection, and how the drug is processed in the body when given in this new way.
This sterile 0.9 % saline solution is given as an injection directly into the knee joint, using a syringe to deliver a small volume of liquid. It is a well‑known, widely used medical product that serves as a solvent and diluting agent and is often employed as a placebo in clinical studies. Its purpose is to provide an isotonic environment without any active drug effect, so it does not treat disease itself. Pharmacologically it is classified as a solvent/irrigating solution (ATC V07AB).
Liraglutide is supplied as a clear solution for injection and in this trial is injected into the knee joint with a syringe, delivering a measured dose of the peptide. It is an approved medication that is already used for type‑2 diabetes and weight management and has been studied extensively in many clinical papers. The drug works by activating the body's GLP‑1 receptors, which helps the pancreas release more insulin after meals and reduces appetite, leading to lower blood sugar and weight loss. It belongs to the class of GLP‑1 receptor agonists, a type of peptide hormone analogue.
