What is this trial about?

A plain-language summary of the goals, design and what participants do

This clinical trial is focused on studying the effects of a specific pain management technique following kidney removal surgery in patients with renal cancer. The study will use a method called the Quadratus lumborum block, which is a type of local anesthesia aimed at reducing pain after surgery. The medications involved in this study include Ropivacaine, a local anesthetic, Dexamethasone, a corticosteroid that helps reduce inflammation, and Sodium Chloride, which is used as a solution for infusion.

The purpose of the study is to determine if the Quadratus lumborum block can decrease the need for additional pain relief after surgery. Participants will receive either the active treatment or a placebo. The study will monitor the amount of pain medication needed at various times after the anesthesia is administered, as well as the time it takes before the first pain medication is required. Additionally, the study will assess pain levels both at rest and during movement.

The trial will follow participants through their recovery process to gather data on how effective the pain management technique is in reducing the need for further pain relief. This information will help improve pain management strategies for patients undergoing kidney removal surgery due to renal cancer.

The research process

The trial runs in 5 steps – from screening to follow-up. Each step says what happens and what the team monitors.

  1. Step 1

    Joining the study

Participation begins after meeting the criteria for inclusion, specifically being a patient with renal cancer undergoing laparoscopic radical nephrectomy.

  2. Step 2

    Administration of medications

The study involves the administration of three medications via injection: ropivacaine hydrochloride, dexamethasone sodium phosphate, and sodium chloride.

Ropivacaine hydrochloride is used as a local anesthetic to block pain.

Dexamethasone sodium phosphate is a steroid that helps reduce inflammation.

Sodium chloride is used as a solution for infusion to maintain fluid balance.

  3. Step 3

    Pain management assessment

The main objective is to determine if the quadratus lumborum block reduces the need for additional pain relief after surgery.

Pain management is assessed by measuring cumulative opioid consumption at 1, 2, 4, 8, 12, 16, and 24 hours after the block is administered.

  4. Step 4

    Secondary assessments

Secondary assessments include the time from the block administration to the need for the first pain medication and evaluating pain levels at rest and during movement.

  5. Step 5

    Completion of the study

The study is estimated to conclude by December 31, 2027.

Who can join the trial?

5 criteria

  • Patients must have **renal cancer**. This is a type of cancer that affects the kidneys.
  • Patients must be scheduled to undergo a **laparoscopic radical nephrectomy**. This is a surgical procedure to remove a kidney using small incisions and a camera.
  • Patients must be adults, meaning they are 18 years or older.
  • Both **male and female** patients can participate.
  • Patients should not belong to a vulnerable population, which means they should not be in a group that needs special protection or care.

Who cannot join the trial?

4 criteria

  • Patients who are not diagnosed with renal cancer cannot participate. Renal cancer is a type of cancer that starts in the kidneys.
  • Patients who are younger than 18 years old cannot participate.
  • Patients who are older than 65 years old cannot participate.
  • Patients who are part of a vulnerable population cannot participate. A vulnerable population includes groups of people who may have difficulty giving informed consent or are at higher risk of harm.

Investigated drugs

Quadratus Lumborum Block is a type of regional anesthesia used to manage pain. In this clinical trial, it is being studied to see if it can reduce the need for additional pain relief after kidney removal surgery. This technique involves injecting a local anesthetic near the quadratus lumborum muscle in the lower back to block pain signals from reaching the brain, potentially improving recovery and reducing the need for other pain medications.

What is already known about the treatment

Quadratus Lumborum Block – This is a regional anesthesia technique administered through an injection near the lower back to manage pain. It is currently being studied in clinical trials to assess its effectiveness in reducing the need for additional pain relief after kidney removal surgery. The main therapeutic indication is pain management following renal cancer surgery. The mechanism of action involves blocking nerve signals in the lower back area, which helps in reducing pain perception. It is classified under regional anesthesia techniques.

Investigated diseases

Renal Cancer – Renal cancer, also known as kidney cancer, begins in the cells of the kidney. It often starts in the lining of tiny tubes within the kidney. As the disease progresses, it can form a mass or tumor that may grow larger over time. The cancer can spread to nearby tissues or other parts of the body if not contained. Symptoms may include blood in the urine, a persistent pain in the side, or a lump in the abdomen. The progression of the disease can vary, with some cases remaining localized while others may advance more aggressively.
